The intensity of the acute subjective spiritual experience was evaluated via a modified version of the nine-item Short Index of Mystical Orientation (SIMO) [61] (see [5] for more information). The, growing interest in ayahuasca also points to weaknesses in Western medicine, therapeutic and healing regimes. Most people that attend traditional ayahuasca healing ceremonies are seeking healing for mental health and emotional difficulties [5, 23, 34]. Indeed, some consider that traditional ayahuasca medicine may be a tool to combat the current global mental health crisis. Such consideration by international policy may be described as reciprocal Global Mental Health [8], where not only biological psychiatry is exported to traditional societies, but traditional medicine may be exported in decolonial and non-extractive ways to Western societies.

Experiences Questionnaire–Decentering

This study assessed naïve users of ayahuasca in order to collect data regarding its overall effects on psychiatric condition and psychological status. Since ayahuasca use has become globally popular as a self-help practice, it is important to better understand its potential risks and benefits. General mid-term adverse effects were not observed in this study, although some secondary acute reactions were observed in some individual cases (e.g. anxiety) and will be reported in a separate paper. Additionally, from a clinical point of view, there was a substantial decrease in psychiatric symptomatology after the first use of ayahuasca, which persisted until the 6-months follow-up. These improvements in depression found after performing clinical interviews were also demonstrated by the psychiatric rating scales.

Participants

A total of 57 participants from either location consented after goals and methods of the study were explained. The participants were not affiliated to any ayahuasca religion (e.g., União do Vegetal, or Santo Daime). Researchers at the University of Melbourne used data for a study on ayahuasca dangers from an online global survey carried out between 2017 and 2019, involving 10,836 people from more than 50 countries who were at least 18 years old and had used ayahuasca at least once. Thirty volunteers were included at several ayahuasca ceremonies in the Netherlands (40% males and 60% females). Most participants were from Europe (93.3%) while the rest of the participants were from Asia (3.3%) and North America (3.3%). Their motivation for ayahuasca use included “understanding myself” (73.3%), “solving issues” (20%), and “curiosity” (6.7%). The highest completed level of education was higher vocational education (70%), high school (16.7%), a scientific education (10%), or elementary school (3.3%). Baseline and sub-acute assessments were conducted on-site whereas the 4-week follow-up was conducted online.

During ayahuasca ceremonies, the medicine is brewed into a tea and then prepared and administered by trained shamans. Shaman helpers, who are typically trained, often for years, by shamans, may also be present to assist participants with restroom access, basic shamanic healing practices and supervision. Ayahuasca ceremonies may take place over three or four nighttime sessions, each lasting several hours.

After the space is prepared and blessed by the shaman leading the ceremony, Ayahuasca is offered to participants, sometimes split into several doses. On the question, “how spiritually significant was the experience,” 8 (47.1%) rated it as the most spiritually significant experience of their lives, whereas 4 (23.5%) and 2 (11.8%) rated it as among the 5 and 10 most spiritual experiences of their lives, respectively. People with schizophrenia or other mental health disorders should also avoid taking ayahuasca.
